Creating a Ticket Form

Ticket forms let you collect information from users before their ticket channel is created. Instead of staff asking "what is your order number?" in every ticket, the answer is already there when the ticket opens.

â„šī¸Forms are per ticket type

Each ticket type can have its own form with different questions. A billing ticket might ask for an order ID, while a technical support ticket might ask for a description of the issue.

Creating a Form

Go to Ticket Configuration
From your server dashboard, click Ticket Configuration in the sidebar.

Open a ticket type
Click on the ticket type you want to add a form to. If you haven't created a ticket type yet, create one first.

Go to the Forms tab
Inside the ticket type editor, click the Forms tab.

Add a question
Click Add Question. Enter your question text, choose the input type (short text or paragraph), and set whether it is required.

Add more questions
Repeat for each question you need. You can add up to 5 questions per form — this is a Discord limitation on modals.

Save
Click Save. The next time a user opens a ticket of this type, they will be shown a form to fill in before the ticket channel is created.

How It Looks to Users

When a user clicks the panel button, Discord shows a modal popup with your form questions. The user fills in the answers and submits. TixBot then creates the ticket channel with the answers already included in the welcome embed.

âš ī¸Maximum 5 questions

Discord modals support a maximum of 5 input fields. Design your form to collect only the most essential information — you can always ask follow-up questions inside the ticket.
